Dear Patients and Families,

All of us at the Life Fitness Center hope that you and your families are doing well as we re-enter a social life following a year of Covid.

​

The purpose of this letter is to let you know that after 45 years of practicing group integrative medicine and psychiatry I have carefully decided to stop practicing on Sunday, August 1, 2021. This is necessary so to pursue the other aspects of my life that I have had to put on hold. This is a bittersweet decision for me and I will sincerely miss you and I wish you well. I am still in good health and it is time for me to complete this segment of my life and fulfill my other intentions. I have been honored that you have chosen me to serve you. I hope that I have been of help to you. I have also learned from you.  Life can be challenging.

If you are currently seeing Dr Cheri Ewing, Dr Jennifer Nill, or Dr Jane Mathews, they will continue practicing apart from Life Fitness Centers. Please speak with them regarding their future plans.  Our administrative staff will continue to help you through this transition. We have so far been unsuccessful in finding another psychiatrist to replace me and run the Life Fitness Center.  We will continue looking and welcome your suggestions.

We suggest that you check with your insurance company, your PCP, and your therapist for recommendations for a new psychiatrist. I apologize    for your inconvenience of having to find another good psychiatrist.

We will maintain your medical records for as long as necessary. If needed, we will provide a summary of your record for your next psychiatrist. You will need to obtain, complete, sign, and return to us a release of records form. For your confidential security, we do not release your psychiatric entire medical record to you.

Please be sure that your financial balance with LFC has been completely paid by July 15, 2021.

I hope to see all of you a few times before August 1 to review your progress, provide enough medications, and to have time to say goodbye and thanks.
